醫用硫酸鈣OsteoSet修復頜骨缺損的療效評價_《中國修復重建外科雜志》

【摘要】目的評價醫用硫酸鈣OsteoSet骨移植替代物在頜骨囊性病變切除后骨缺損治療中的效果。方法 2009年12月－2010年5月，收治15例頜骨囊性病變患者。其中男9例，女6例；年齡15～75歲，平均36.6歲。患者術前均攝頜骨全景斷層片（orthopantomography，OPT）測量囊腫大小，范圍為1.5 cm × 1.5 cm～8.0 cm × 3.0 cm。均采用囊腫刮治術加OsteoSet骨移植替代物移植術進行治療，術中囊腫切除后骨缺損范圍為1.5 cm × 1.5 cm × 1.5 cm～8.0 cm × 3.0 cm × 3.0 cm，OsteoSet骨移植替代物植入量為2～15 mL。通過影像學方法評估OsteoSet骨移植替代物的成骨效果。結果術后病理檢查提示：根尖周囊腫7例、牙源性角化囊腫5例、含牙囊腫3例。15例均獲隨訪，隨訪時間6～12個月。13例患者切口Ⅰ期愈合；2例術后引流時間延長，分別為5 d和7 d，經加壓包扎處理后切口愈合良好。1例術后1個月復查出現腫脹，無感染癥狀；無1例出現術后感染和排斥反應。所有患者術后1 d均行X線檢查，見材料將骨缺損充填良好；術后1個月頜骨OPT復查發現植入的OsteoSet顆粒吸收近半；術后3個月植入的OsteoSet顆粒全部吸收，原囊腔低密度影面積明顯變小，囊腔內高密度影增加明顯，囊腔邊界模糊；術后6個月原囊腔內影像密度與正常骨組織密度差別不明顯，囊腔邊界消失，新骨基本充滿缺損區。結論醫用硫酸鈣OsteoSet骨移植替代物是一種理想的骨缺損充填材料。
