人工髁突假體在顳下頜關節重建中的臨床應用_《中國修復重建外科雜志》

【摘要】目的探討人工髁突假體應用于顳下頜關節重建的可行性及臨床治療效果。方法 2005年1月－2010年1月，對10例（11側）患者應用人工髁突假體行顳下頜關節重建手術。男7例，女3例；年齡40～68歲，平均50歲。左側7側，右側4側。下頜骨腫瘤7例，病程9～24個月，平均15個月。雙側髁突囊內骨折3例，其中交通事故傷2 例，高處墜落傷1例；傷后至入院時間分別為2、3、2 d ；骨折按照Neff等的分類法：M型及A型1例，M型及B型1例，余1 例一側為M型，一側為髁突下骨折。結果術后切口均Ⅰ期愈合，無相關并發癥發生。患者均獲隨訪，隨訪時間1～ 4年，面部對稱，咬關系良好；張口度22～38 mm，平均30 mm。雙側顳下頜關節無彈響及疼痛，腫瘤無復發。一側人工髁突假體就位角度稍差，其余人工髁突假體位置良好，與關節窩和關節結節的關系正常，能正常飲食。結論人工髁突假體應用于腫瘤術后顳下頜關節重建，效果良好；應用于髁突囊內骨折的治療，其臨床效果待進一步驗證。
